2004 Tahoe Rear Washer Hose Broken

The rear washer hose on my Tahoe is cracked in two near where the wiper arm connects to the exterior of the lift gate. Can I fix this without having to remove the whole back door panel? If not, how do I fix it in general?

Heat shrink? Or buy a new little house from aute parts store.
