How do I find out which size Disc Brakes do I have ?

Front 12.01 in.; Rear 12.99 in., 4-wheel set or
Front 12.01 in.; Rear 12.76 in., 4-wheel set

I'm getting ready to replace my disc brakes and pads, but I get the question of the rear rotor? How can I find out?

I have a 2000 yukon xl SLT with 4 wheel drive.
