There's a few companies that do it, but it takes a lot of wiring and programming to make the d-max computer and tahoe BCM work together. The body lift is a requirement to make room under the hood/trans tunnel, but still expect to make a custom trans crossmember and the like.
You should also look at SFA or at least 2500 front end. That fabtech will drop a lot with that much weight, and those components aren't designed for a 800lb motor not to mention 550+ lb ft of torque. You will want a 14blt FF for the rear end also, as the 10blt will fail with a 5.3 and 33's commonly.
There are tons more details to making it a reliable, driveable vehicle. If you don't have a lot of fab skills and a buddy with a tech II I can't imagine you'd be saving much by not having someone who has this down do it for you. At least you already have the vehicle and motor, so you're only going to have to pay for labor and the smaller items.
