if it has been sitting for a while and at the age of that engine it's probably going to smoke a bit until it burns off whatever has made it past the rings & seals, then it may blow a small smoke cloud at startup each time it has sat for a more than a few hours and that's normal, if it does then you may have to keep a quart of oil around to keep it topped off once every week or two.

have you checked the typical tune up components yet? advice on the smoke and such could be accurate but the stumble could just be tune up related. also if its running real badly with the stumble you can get excessive fuel getting blown through the exhaust which can also cause some smoke. in a few minutes it should have registered a cel with readable codes if there's other issues it can see. also sitting a long time could have built up excessive moisture in the distributor cap which can cause it to run rough. i cant remember exactly when the 350 went to mpfi is yours a tbi or mpfi? the tbi units can start leaking around the injectors and fuel bowl. it will disrupt the fuel spray pattern and add extra fuel the computer cant account for and will also cause a rough running symptom.

I bought the truck 3 years ago. Originally my plan was to do an ls swap but I never got around to it and it just sat. I did the basics: engine oil and filter, tranny fluid and filter, transfer case fluid, (reason I got the truck so cheap was because t-case was leaking), spark plugs, pcv valve, air and fuel filter. I'm thinking it might just be burning up stuff it accumulated while sitting. Today I used throttle body cleaner and this orange rusty looking stuff came out the tailpipe. Fingers crossed hoping its this and not something worse.IMG_20200718_181625.jpg

That's just rusty water.

Keep an eye on the oil and the coolant levels and hopefully you will be fine.
