Looking at double DIN receivers on Crutchfield's website it says for many that the rear metal bracket has to be cut to make room. Is this really necessary and how difficult and/or how much of a PITA is it to do it?

The depth of certain screens requires this to be done if you want it "Flush" within your dash. Pretty easy to do.

With most dd models, you'll have to remove that bracket if you are fitting it into a 00-02 model. 03+ has room for most dd. I havent had to modify the dash cavity in any 03+ truck for any dd deck, only previous models.
