Here is the guily guys, the one thats in 2 pieces, the porcelain part came out in the plug boot, and the plug thats still together has a hairline crack around the porcelain, that was cylinder 3 and 4! 
Never in my life have i saw them crack like that all of the sudden! I have seen them crack being put in or taken out, but not a few thousand miles down the road, but hey its fixed now, plus a new fuel pump and fuel filter! Thanks again everyone for all the information!

I was about to say its the plugs still i saw this reply, but anyways the samething happened to my 6.0 when i changed the plugs, 1 was bad
