If two tires of different brands are the same size, but have different load index, does that mean the tire with the higher load index use a stiffer sidewall or just general stiffer construction?

It’s pretty interesting. I put 2 new tires on the front on my RV, which is just an E450 chassis. They’re light truck tires designed to have 80 psi cold. I couldn’t believe it but it needs that pressure to carry the load. Their load rating is E.
