Pushrod failure

What gives , what could cause this ? Pushrod where it meets the rocker arm got demolished . Removed pushrod from cyl8 after hearing some very loud lifter tick. After removing pushrod used borescope to look inside that lifter and lifter next to it looks about the same doesnt look or feel seized although I had the motor cranked while holding both pushrods in by hand felt about the same but the pushrod that was bad didnt nearly get enough oil on it like the one next to it. Could the lifter have debris stuck inside ?
Ordered 1 new rocker arm and 1 replacemnt 7.850 BTR chromoly pushrod. For now I just threw in my oem stock pushrod and its runing perfectly fine no lifter tick , nice and quiet.

If I did have a bad seized lifter I would still be getting lifter tick with the temp pushrod i threw in there wouldn't i ?

Threw a bottle of liqui moly hydraulic lifter additive in the engine to see if maybe if there is debris in lifter it will clean it up idk.

It could be just a failed pushrod. Are all lifters oiling at the rocker with the engine running?
I actually removed the ign coil fuses and watched the rockers move without the engine running , maybe I should've done that

With it running looking at the rocker arms you can see if they are oiling or not. If 1 or more is not oiling you have problems.
Spinning with the starter should also get oil to the rockers but that does not always work.

Spit balling here. That push rod looks like the lifter cashed out. Side note, prime and or soak new lifter.
