Rear HVAC broken blend door

Today I pulled the interior trim to swap the blend door motors on my rear HVAC in my '04 Yukon. Ive found that in addition to the motor not working, the temp blend door is actually broken and dangling in the housing. Does anyone have a part number, link, or anything else to help me fix this? I really appreciate anything that helps.

I know this thread is a year old, but did you get it fixed? Thought my problem was the motor but went to replace it and found broken door instead. Did you come up with a part #?
Have not been to the dealer yet, but tried Rock Auto and the other usual suspects with no luck. The closest dealer is 1.5 hours away. Sorry, 2006 Tahoe
