Thread necro, but I thought I would post an update. Turns out the passenger side exhaust manifold gasket was leaking due to a broken bolt (at cylinder #8 of course). The exhaust finally breached the gasket completely, which exposed the problem. New bolt and new gasket, and she's quiet a church mouse.
