Where can I access the line out ports to connect a sub in the rear? Can I leave the factory center console sub connected and add an additional sub in the back? Or do I need to disconnect it? I haven't seen any good videos or guides on where people are running the cables

All information you need to install sub in stock rear quarter panel is in above link. I recently did sub install in my Tahoe HC using above the info. No need to disconnect center console sub. You will need sub box, subwoofer, amp, LOC, RCA cable, amp wire kit, T harness, ANC bypass, speaker wires, & remote turn on wire. I also added sound deadening material & subwoofer remote for my install.
Sub signal is best accessed off the Bose amp...you will need T harness to do this....just plug n play. I have an extra T harness if you're interested...$60.
