I'm going to replace the PCV valve in my 02 Tahoe 5.3 Flex, pretty sure it's never been done in 295K Miles...

The one stores match to it is the spring type, but I've read some stuff saying I should switch the the newer fixed orifice type? Others said that requires some modification other than just replacing the valve. What should I do?

New style is built into the valve cover, requires changing the whole cover. Just stick with the oem type and change it and the grommet (its probably not sealing well after that many miles) unless you are already changing valve cover gaskets.
