Pretty sure it's an SSV and, with those hours and the mileage, it's like its only purpose was to make a few highway runs ever so often. Lots of PPVs/SSVs can have low miles, but they tend to have high engine hours from extensive idling. Yours has low miles AND hours. No DIC switches, no center console, no body molding and those wheels all point to it being an SSV. Cool score.
For the leveling kit, hit up Amazon. All they are is a spacer to go under the bottom of the strut mount or on top of the strut. Measure how yours sits now and determine how much you'd wanna pick it up in the front. Use a jack under the cross member to lift it 1", step back and see what you think then raise it another inch and have another look. You can get a 1.5" if that's what you decide on. Oh- measure from the underside of the fender lip to the center of the wheel, on the center cap.
I got a 2" lower strut spacer kit made from billet aluminum for $22 on Amazon. I used my own Grade 8 bolts. It's no longer available but here's the same but with better hardware included:

Also, pretty much all of these SUVs will be lower on the driver's side. Sometimes it's an 1/8" difference, sometimes it's around 3/4". If it were me, and because these kits are so cheap, I'd get a kit of two different sizes to have a little more lift on the driver's side to level it. Or, if you have the means, get one kit that will lift the driver's side the amount you want and have the spacer for the passenger side shaved down accordingly so it's level.
