You just reuse that ground bolt in place of one of the Dorman bolts. There is a separate clamp that works for the drive's front/passenger rear sections of the manifold. Remember, the cyliner heads are identical, so front-left of one side equals rear-right on the other.

I used both styles of clamps on my last engine, as it had a few broken manifold bolts, and needed help sealing up the leaks.
