You just reuse that ground bolt in place of one of the Dorman bolts. There is a separate clamp that works for the drive's front/passenger rear sections of the manifold. Remember, the cyliner heads are identical, so front-left of one side equals rear-right on the other.

I used both styles of clamps on my last engine, as it had a few broken manifold bolts, and needed help sealing up the leaks.

Throw away screw extractors. I've seen many many cases where they break off. Your fortunate some is sticking out. A friend of mine owns a BMW Motorcycle dealer. Here is their remedy. Weld a washer onto the extractor, weld a nut on to the washer, back it out. Try to do it as soon after the weld as possible as the heat will make it easier to back out. If the bolt is too tough to come out on its own, it's too tough for a screw extractor

I've seen people put a short pipe over the broken bolt and then weld to the bolt inside the pipe - the pipe will protect the surrounding material and give you something to grab onto as well...
