Wanted to close the loop for everybody here: it was the intake manifold gasket that failed. Lean fuel reads. New gaskets and it’s running great. Hate paying for work but it wasn’t too bad and fixed my issue.
I think on these older higher mileage trucks the intake manifold gasket should probably be replaced for preventative maintenance because those seals dry out and shrink. I had just done the oil pressure sending unit because of leaking (without removing the manifold) and I had the guy do the lifter valley gasket while he was in there. I would recommend new owners do all 3.
