They wouldn't sell one manual trans to every 100,000 autos, especially in a triple digit soccer mom suv.
That's why they don't make them anymore. Hell, they don't even put them in workhorse pickup trucks anymore.
They still make 'em. They still put 'em in Mexican 2500 3500 4500 & 5500 pickup trucks. They is Tremec.

Manual transmissions are not as efficient as our new automatics. Squeezing every last drop of MPGs out kinda rules them out for USA customers too.

For normal driving routines, this may be true.
However, there are things clever drivers can do with manuals that are impossible with automatics.
Think hypermilers.

I regularly corresponded with a guy on the Impala SS Forum (funkhoss) who installed a junkyard NV4500 into his LT1 wagon.
He regularly got upwards of 40MpG with it, if his considerable records stack is to be believed.
(THE popular mid-90s LT1 manual is STILL the T56, but it is too weak for our trucks.)

OE used to be the New Venture 4500 for medium duty powertrains.
South of the border, the Tremec TR4050 has superseded NV4500 as the OE manual option.
The rest of the world still prefers a manual transmission option in most vehicles as well.

If I could afford a Tremec TR4050, I'd swap out my 4L60E in a heartbeat.
