Had two shops check everything over before a big road trip last summer and they said all seemed fine. One of the shops was the guys who had replaced the hose I mentioned above plus a second one who put a new coolant tank in as suggested.

Everything was not ok and there was a big cloud of steam at one point. We opened the hood and found it was leaking fairly rapidly near the firewall right. I tugged on the connection and it came apart quite easily!

The internal retaining plastic clip had been partially crushed during install so I gently made it look right and reconnected. Problem solved.

So the root cause was a botched installation but I'm surprised no one could find it proactively, especially since I suggested they check the firewall T as suggested by @Blackcar above.

Car is generally running well so I think the engine must be fine. Still need to get the head gasket done and the transmission does feel a quite rough shifting at low speeds in traffic so leaning towards getting it replaced.

Thanks for everyone's advice and help!

Dodged the bullet. If that’s a truck you’ll regret investing in further deferred maintenance, it’s time to sell it.
