Surging/jerking at cold start??

I have 2010 AWD Denali and whenever the engine is cold I get a horrible surging/jerking when I press the accelerator from a dead stop. Once the motor has had a few minutes to warm up, I do not get the severe surging/jerking. However, it's almost impossible to launch smoothly from a dead stop. I get a rather abrupt clunk in the driveline that will snap your neck back. Have to really focus on not depressing the accelerator too hard too fast to launch smoothly....

Thoughts welcomed.

Anyone?

I've tried running a good fuel system cleaner thru a couple tanks and problem still exists. MAF or throttle body? Thoughts?

Is it storing any codes? Which codes?
Are you running a tune? Did it start after you were tuned?
Have you checked for vacuum leaks?
How long since it's bee serviced (ie tuneup)
Have you checked the fluid level in your transmission? What is its condition?
When was the transfer case last serviced? Front and Rear differentials?
Have you cleaned your throttle body? IAC?
Is it hard to start?

Thanks Jason for the reply. Turned out to be a broken motor mount. I had a tune up done at ~58k miles and had it done again last week. Truck now top runs fantastic with no jerking at cold start and smooth launches with no clunking. Very happy with the exception of the bill.....
